For elder-care media buyers
Put the care context behind the campaign.
Connect the correct facility, dated care findings and reported staffing. Build a campaign brief around a specific care question, then test what happens through qualification and signing.
Build the brief
Separate the care themes. Ask better questions.
Pressure injuries, supervision and assisted-living protections need different evidence. A low rating or a staffing threshold cannot do all the qualification.
1,034 retained F0686 citations.
The retained CMS cycles contain 1,034 pressure-ulcer-care citations, including 86 coded G–L. Those codes span actual harm and immediate jeopardy; jeopardy can involve threatened harm.
Rolling retained survey cycles, not annual injury counts, unique residents or legal cases.
Build a care-records information test.
Compare a pressure-injury records checklist with a general elder-care page under comparable eligible query traffic. Staffing is context, not an intake pass/fail rule.
Qualify the inquiry
- Resident protection status and care setting
- Exact facility license/CCN and stay dates
- Known risks, care plan, injury and relevant omissions
Measure: Attorney acceptance, rejection reasons and signed matters after lag.
The historical Pine Creek quarter recovered for the selected pressure-injury case has no occupied day below eight reported RN hours. Exact claimant stay dates and causation remain unverified.
The evidence register
A larger panel. A clearer identity.
Keep the record grain, date window and missing information beside every claim. These are source-backed research observations.
| Observation | What was counted | Boundary |
|---|---|---|
| California staffing panel207,066 facility-days | Reported daily paid hours and MDS census across 2026 Q1/Q2. Q2 released October 28, 2026. CMS daily nurse staffing | Reported paid hours are not verified bedside attendance or an abuse finding. |
| Q2 directory reconciliation1,146 of 1,146 | Quarterly facilities matched to the current cleaned CMS directory by exact CCN. CMS provider directory | A current directory match does not establish historical operator responsibility. |
| Weekend comparison1,141 of 1,146 | Facilities with lower weekend than weekday core HPRD; median facility ratio 0.875. CMS daily nurse staffing | A common weekend dip has little discrimination alone and does not justify ad dayparting. |
| Same-quarter counterexample49 of 50 | Matched facilities with retained G–L citations in April–June 2026 that had no reported zero-RN day. CMS health citations | Ecological co-observation only. Not classifier performance, causation or a resident-level match. |
Why reported staffing is not verified attendance or a neglect label
HHS OIG’s June 2026 audit found unsupported RN hours in its March 2024 sample and estimated about 5% unsupported hours within that sampling frame. Do not apply that percentage as a correction to all current California records. Keep reported staffing, care findings, resident-level proof and legal conclusions separate.
Put the research to work
One clear question.
One testable change.
Choose an evidence-backed topic your counsel accepts. Keep the campaign comparison narrow enough to explain the result.
Define the evidence context
Pin the source, date window, record grain and exact entity. Preserve contradictory facts and missing information.
Change one part of the campaign
Compare a specific information page or qualification flow with a concurrent control. Keep geography, intake coverage and the outcome definition comparable.
Follow the matter through intake
Deduplicate inquiries, record attorney acceptance and allow time for signing. Reallocate only when mature economics, uncertainty and capacity support the decision.
The buying decision
Work back from
a signed matter.
Use your firm’s economics to calculate an acquisition ceiling. A public settlement amount cannot supply these inputs.
Inputs stay in this page’s memory. They are not sent to us or an ad platform.
Add all five inputs to calculate a ceiling.
Calculation and assumptions
Target CPL = signed-per-lead × expected contribution × (1 − retained share) − intake cost per lead. Target CPC = lead-per-click × target CPL. Negative ceilings display as zero. When signed-per-lead is zero, cost per signed matter is undefined.
Contribution incorporates expected noncollection, case costs and timing. Fixed overhead, uncertain conversion rates and additional acquisition costs need separate review. This calculator does not estimate legal merit, recovery or incremental return.
Sources and scope
Open the evidence.
The panel covers CMS-certified nursing homes. RN includes director and administrative RN hours; core adds defined LPN and CNA categories. HPRD is summed hours divided by occupied resident-days. Assisted-living history, resident-level outcomes and campaign economics remain separate evidence gaps.

- CMS daily nurse staffing
Verified CA-filtered 2026 Q1/Q2 pages, unique CCN/date grain and pinned raw hashes. Hours are paid/reporting measures, not observed bedside care.
- CMS health citations
October 2026 retained citation cycles, filtered to California facilities. G–L spans actual harm and immediate jeopardy.
- CMS provider directory
Current roster supports exact-CCN reconciliation; it does not replace license/operator history.
- California facility search
Assisted-living licenses form a separate population; absence of integrated findings does not mean no complaints.
- HHS OIG staffing audit
June 17, 2026 audit of March 2024 reported RN hours; its sampling-frame estimate is not a universal current correction.
- California WIC 15657
Enhanced remedies have specific culpability, proof and employer requirements.
- California WIC 15657.02
The 2026 spoliation provision requires prescribed judicial findings and applicability; missing records alone are insufficient.
The download includes the public claims, sources and four campaign hypotheses. Raw narratives, personal information and private research archives are excluded. Retrieved documents and current snapshots cannot establish what was publicly known before a historical incident.
Legal qualification and advertising boundaries
Enhanced elder-abuse/neglect remedies require the protected-person and care relationship, actual omissions, harm, causation and the applicable culpability/proof standards. A citation alone does not establish civil liability. Professional negligence and financial abuse have different branches. Missing records alone do not satisfy the statutory judicial spoliation exception.
Health and abuse-related promotions have platform targeting and data-use restrictions. Use eligible query/context campaigns and review each creative, landing page and measurement method. Sensitive intake belongs in restricted first-party systems. Public records are context, not a person-level audience list.

What would establish acquisition performance?
A dated keyword/auction export, mature deduplicated intake cohorts, attorney acceptance and signed outcomes, actual firm contribution inputs, and a prespecified concurrent comparison. Track pending matters and conversion lag. Public counts supply none of those ad-performance inputs.
No campaign was launched for this research. CPC, acquisition cost and incremental return remain unmeasured. Qualified and signed-matter details must not be exported to an ad platform without a method-specific policy and privacy review.
Start with a buying decision
Bring a care question. Trace the evidence.
Bring the geography, topic and decision you want to test. We’ll discuss an aggregate brief with dated sources and clear limits.
Public records stay public. A research brief does not promise legal eligibility or campaign results.
